Results: The patient retained the affected limb, albeit with some functional limitations, following early identification, timely surgical debridement, broad-spectrum anti-infective therapy, and comprehensive nursing care. Conclusion: The early clinical manifestations of necrotizing fasciitis are often subtle, and the disease progresses rapidly. This necessitates heightened vigilance among medical staff in all healthcare settings. Its core mechanisms involve multi-level regulation of free fatty acid (FFA) metabolism and chronic low-grade inflammatory states (represented by tumor necrosis factor-alpha, TNF-α). This paper systematically reviews the direct impact of LSG on FFA dynamics (including lipolysis, tissue uptake, and oxidation), as well as the molecular pathways through which it indirectly regulates TNF-α by reducing adipose tissue inflammation, improving intestinal barrier function, and modulating epigenetic modifications (such as SCD gene methylation). Postoperatively, FFA and TNF-α form a bidirectional promoting feedback loop. LSG effectively breaks this vicious cycle of mutual promotion between the two under obese conditions by reducing FFA levels and inhibiting TNF-α expression. The levels of serum IL-33, TNF-α, IL-6, and HMGB1, as well as the expression of miRNA-155 in extracellular vesicles (EVs), were dynamically monitored at multiple time points (0 h, 6 h, 12 h, 24 h, 3 d, 5 d, 7 d, 14 d) after admission. Abdominal CT radiomics analyzed the texture characteristics of pancreatic necrosis, and stool samples collected at admission were metagenomic sequencing of the gut microbiome. The Acute Pancreatitis Severity Bedside Index (BISAP) score is calculated within 48 hours of admission. Multivariate regression analysis assessed the independent effects of various factors on the prognosis of mortality groups. <em>Results</em>: Serum IL-33 and TNF-α levels in SAP patients were significantly higher than those in MAP patients (<em>p </em>&lt; 0.05) at all time points, peaked on day 3, and decreased with treatment. The levels of these cytokines in patients with SIRS were also higher than in patients without SIRS (<em>p </em>&lt; 0.05). The serum IL-33, TNF-α levels and BISAP scores in the mortality group were higher than those in the survival group (<em>p </em>&lt; 0.05). Multivariate regression analysis showed that serum IL-33 (OR = 3.21, 95% CI: 1.12–9.23, <em>p </em>= 0.03), TNF-α (OR = 4.05, 95% CI: 1.37–11.96, <em>p </em>= 0.01), and BISAP score (OR = 5.67, 95% CI: 1.83–17.54, <em>p </em>&lt; 0.01) were independent prognostic risk factors. According to the SMART principle, various work assessment indicators&nbsp;were quantified. This involved setting clear work goals, diversifying work methods, personalizing work methods, standardizing workflows, and using numerical assessment indicators. Regular supervision, inspection, feedback, and improvement mechanisms were implemented. <strong>Results</strong><strong>&nbsp;</strong>The implementation of the target management model has made the work effectiveness of clinical pharmacists visualized.&nbsp;There were more than 200 annual consultations and multidisciplinary team (MDT) cases, with an opinion adoption rate of 90.2% and a patient improvement rate of 80.6%. More than 1500 rational drug use interventions were conducted, with a suggestion adoption rate of 83.5%. In terms of pharmaceutical indicators control.&nbsp;The intensity of antibacterial drug use in 2024 (without CMI adjustment) was 30.07 DDDs, significantly lower than the 2023 value of 33.54 DDDs, and also significantly lower than the provincial average (32.87 DDDs) and the average for hospitals of the same level (32.49 DDDs).&nbsp;The daily usage of intravenous infusion per bed for hospitalized patients was&nbsp;2.09, a decrease from 2.15 in 2023, significantly lower than the provincial average of 2.71 and the average of 2.56 in hospitals of the same level. The amount of the second batch of national key monitoring drugs accounts for the value&nbsp;was&nbsp;6.48%, significantly lower than the provincial average of 8.27% and the same level hospital average of 8.82%. In terms of chronic disease pharmaceutical management, taking the pharmaceutical management of patients with chronic heart failure as an example, the usage rates of RA inhibitors and β-blockers for heart failure in the management group were 87.88% and 80.81%, respectively, significantly higher than those in the control group (62.22% and 65.56%). Heart rate in the management group (69.54 ± 10.68&nbsp;times·min<sup>-1 </sup>) was significantly lower than in the control group (80.04 ± 17.68 times·min<sup>-1 </sup>) (P&lt;0.001). The low-density lipoprotein (1.69 ± 0.57&nbsp;mmol·L<sup>-1</sup>) was significantly lower than the control group (1.95 ± 0.77&nbsp;mmol·L<sup>-1</sup>) (<em>P&lt;0.001</em>), and the 1-year readmission rate was 47.47%, significantly lower than the control group 56.67%. There have been few reported cases among sedentary individuals. This case report details the experience of a 42-year-old sedentary woman who fainted during a treadmill stress test. Despite the absence of abnormalities in baseline cardiac and neurological evaluations, the patient exhibited sinus arrest (lasting 5–12 seconds) with significant ST-segment depression during haemodynamic collapse. Comprehensive assessments, incorporating coronary angiography, echocardiography, cranial computed tomography (CT) and biochemical testing, excluded the presence of structural or ischemic heart disease, arrhythmogenic syndromes and cerebrovascular disorders. A Calgary Syncope Symptom Score of 3 confirmed the diagnosis of VVS, a diagnosis that was further substantiated by the patient's symptoms resolving spontaneously when she was positioned supine. This case demonstrates that exercise-induced syncope can occur in individuals who are physically unfit and have no cardiac abnormalities. Transient ST-segment changes in such cases reflect autonomic nervous system dysfunction rather than myocardial ischaemia. It is incumbent upon clinicians to consider a neurocardiogenic mechanism in sedentary patients presenting with exertional syncope despite a negative standard cardiac evaluation.</p> Yu Zhang, Jian Huang Copyright (c) 2025 Author(s) https://www.bbwpublisher.com/index.php/JCNR/article/view/11679 Wed, 22 Oct 2025 00:00:00 +0800 A Seven-Year Surveillance of Epidemiological Trends of Serratia marcescens with Different Infection Types in a Tertiary Hospital in China https://www.bbwpublisher.com/index.php/JCNR/article/view/12271 <p><em>Objective:</em> To explore the trend of detection and antimicrobial resistance of <em>Serratia marcescens</em> with different infection types for 7 consecutive years, to provide a reference for future studies for the control of <em>S. marcescens</em> infections and a rational selection of antibiotics.<em> Methods:</em> <em>S. marcescens</em> isolates were collected from 2014 to 2020, and the trend of detection and antimicrobial resistance were analyzed according to different types of infection. <em>Results:</em> For 7 consecutive years, the data showed that patients with <em>S. marcescens</em> infections were mainly from the intensive care unit (ICU) (384 isolates, 40.98%), and the isolates recovered were mainly from sputum samples (743 isolates, 79.30%). The number of isolated strains increased every year, and the average rate of detection ranged from 0.60% to 0.80%. The detection rate of <em>S. marcescens</em> with hospital-acquired infections (HAI) showed a downward trend and that of <em>S. marcescens</em> with colonization showed an upward trend. The detection rate of multidrug-resistant <em>S. marcescens</em> fluctuated between 8.33%–16.89%. The resistance rate of <em>S. marcescens</em> to piperacillin was 17.0%–29.06% and the resistance rate to piperacillin tazobactam was 2.95%–13.13%. For cephalosporin antibiotics, the resistance rates of <em>S. marcescens</em> to cefuroxime and cefazolin were &gt; 99% and the resistance rates to ceftazidime and cefepime were &lt; 13%. The resistance rate of <em>S. marcescens</em> to aminoglycoside antibiotics, especially amikacin, was the lowest. The resistance rate of <em>S. marcescens</em> with community-acquired infections (CAI) to carbapenems was higher than that with HAI and colonization. <em>Conclusion:</em> The different infection types of <em>S. marcescens</em> have different detection and epidemic trends. The experimental group received comprehensive nursing intervention of developmental care during the nursing process; the control group adopted the traditional conventional nursing model. <em>Results</em>: There was no statistically significant difference in various physical indicators between the two groups of premature infants before nursing intervention. Sixty-six patients were selected as the research subjects by the random sampling method and divided into a control group and an observation group. The patients in the control group were treated with ribavirin injection and oral lysine-inositol-vitamin B12 solution, while the patients in the observation group received the treatment of the control group combined with recombinant human interferon α2b nebulization inhalation. The clinical effects of the two groups were compared to determine the clinical efficacy of the two treatment plans. The total effective rates of treatment were 78.79% in the control group and 90.91% in the observation group. The research confirmed that the clinical effect of recombinant human interferon α2b nebulization inhalation in the treatment of hand, foot, and mouth disease in children is remarkable. The control group received routine nursing, while the observation group received orthopedic nursing clinical pathway on the basis of traditional nursing. We analyzed and scored the nursing satisfaction and nursing effect of the two groups. <em>Results</em>: Compared with the control group, the observation group had a higher total nursing satisfaction rate, shorter time to get out of bed, shorter hospital stay, and lower hospitalization cost. The observation group used modified incision, while the control group used traditional triangular incision. The operation time, intraoperative visual analogue scale (VAS) pain score, pain VAS score on the 1<sup>st</sup>, 2<sup>nd</sup> and 3<sup>rd</sup> day after operation, facial swelling on the 2<sup>nd</sup> day after operation, mouth opening limitation on the 2nd day after operation and the incidence of dry socket after operation were recorded and compared between the two groups. <em>Results</em>: There was no significant difference in operation time between the two groups (<em>p </em>&gt; 0.05). The VAS score of pain in the observation group was significantly lower than that in the control group at each time point during and after operation (<em>p </em>&lt; 0.05). On the second day after operation, the facial swelling and mouth opening limitation in the observation group were significantly lighter than those in the control group (<em>p </em>&lt; 0.05). Comparisons of liver stiffness values also showed that the severe group &gt; moderate group &gt; mild group &gt; control group, with <em>P</em> &lt; 0.05. Pearson correlation analysis revealed a positive correlation between liver stiffness values and body mass index, triglycerides, total cholesterol, low-density lipoprotein, fasting blood glucose, and glycosylated hemoglobin. They were divided into a control group (90 cases, using traditional scoring methods to predict prognosis) and an observation group (90 cases, using a multivariable model based on machine learning algorithms to predict prognosis) according to the random number table method. General information, laboratory indicators, and clinical interventions were collected. Various models such as Random Forest (RF), Support Vector Machine (SVM), and Logistic Regression (LR) were established. The model performance was evaluated using ROC curve, AUC value, accuracy, sensitivity, and specificity. <em>Results</em>: The machine learning models performed better than traditional scoring methods in predicting the 28-day mortality rate of septic children. Among them, the RF model achieved an AUC value of 0.921, a sensitivity of 85.6%, and a specificity of 88.1%, which were significantly higher than the PIM3 score (AUC 0.762). This study adopted a correlational research design to determine the association between self-care readiness (independent variable) and self-management (dependent variable). This study surveyed respondents using a three-part questionnaire, and their scores were recorded using the Demographic Profile Questionnaire, Transition Readiness Assessment Questionnaire (TRAQ), and Diabetes Self-Management Questionnaire (DSQM). The data were analyzed using statistical tools such as frequency, percentage, mean, standard deviation, Pearson R and hypothesis testing. The results showed a correlation between independent self-care and self-management in diabetic patients. These findings indicate that the respondents generally reported moderate to high levels of readiness and self-management, with more variability noted in self-management scores. The Benefit Finding Scale (BFS), Hope Level Scale (HHI), and Quality of Life Questionnaire-Core 30 (QLQ-C30) were used to compare and analyze the scores of the two groups before intervention, 3 months after intervention, and 6 months after intervention. <em>Results</em>: There was no statistically significant difference in BFS, HHI, and QLQ-C30 scores between the two groups before intervention (<em>P </em>&gt; 0.05). Both BFS and HHI scores increased in the two groups at 3 and 6 months after intervention compared to before intervention (<em>P </em>&lt; 0.05), and the observation group had better BFS and HHI scores compared to the control group (<em>P </em>&lt; 0.05). Before intervention, there was no significant difference in QLQ-C30 scores between the two groups in all dimensions (<em>P </em>&gt; 0.05). Data comparison at 6 months after intervention showed that the observation group had better scores in all dimensions of QLQ-C30 than the control group (<em>P </em>&lt; 0.05). <em>Conclusion</em>: Nursing intervention based on the theory of Gratitude Extension-Construction can effectively improve the benefit-finding ability of postoperative breast cancer patients, enhance their level of hope, and improve their quality of life. The results of myopia and visual fatigue improvement experiment showed that the prevalence of myopia in the control group was 15%, and the prevalence of myopia in the experimental group was 5%. The prevalence of myopia in the experimental group was significantly lower than that in the control group. The ocular symptoms of the experimental group were better than those of the control group in the improvement of fatigue, dry eyes, red blood stains and other symptoms. The patient satisfaction score of the experimental group was 95.5%, which was significantly higher than that of the control group. The expression level of circMGAT5 was detected by quantitative real-time polymerase chain reaction (qRT-PCR). The proliferation and invasive abilities of cancer cells were determined using soft agar colony formation assay and Transwell assay, respectively. The expression of matrix metalloproteinase-2 (MMP-2) at mRNA and protein levels was detected by qRT-PCR and immunofluorescence, respectively. <em>Results:</em> CircMGAT5 level in the circMGAT5-transfected group was significantly downregulated in a concentration- and time-dependent manner (<em>p </em>&lt; 0.005). Soft agar colony formation assay exhibited that the number of colonies formed in the 6.25, 12.5, and 25.0 nM siRNA groups was significantly reduced in a concentration-dependent manner (<em>p </em>&lt; 0.005). Eligible studies included English-language original research and review articles published between 2005 and 2025. Bibliometric analyses were performed using VOSviewer and CiteSpace to evaluate publication trends, co-citation networks, author collaborations, institutional and country partnerships, and keyword co-occurrence. <em>Results</em>: A total of 570 articles (476 original research papers and 94 reviews) were included. The annual number of publications increased exponentially, peaking in 2020. Co-citation analysis identified Wakefield (2005), Naredo, and Filippucci as foundational contributors. Author collaboration networks highlighted a strong European core centered on Italy and Spain, while institutional and country analyses revealed a “Europe–North America” dual-center pattern with growing contributions from China. Keyword co-occurrence analysis revealed three main research pillars: inflammatory arthropathies, sports-related injuries, and degenerative conditions, with emerging interest in advanced imaging techniques and artificial intelligence. <em>Conclusion</em>: T MSK US research has demonstrated sustained growth and diversification over the past two decades. This study aims to analyze the disease burden of knee osteoarthritis, attributable risk factors among Chinese residents from 1990 to 2021, and predict the disease burden trend for 2035. <em>Methods</em>: Data related to knee osteoarthritis in China from 1990 to 2021, including the number of incident cases, incidence rate, number of prevalent cases, prevalence rate, and years lived with disability (YLDs), were collected from the Global Burden of Disease Study (GBD2021) database. Joinpoint regression analysis was used to assess time trends, and the Bayesian-Age-Period-Cohort (BAPC) regression model was employed for future predictions. <em>Results</em>: From 1990 to 2021, the number of incident cases of knee osteoarthritis among Chinese residents increased from 3.65 million to 8.51 million, a rise of 133.16%, with an average annual increase of 3.15%. The incidence rate increased from 310.33 per 100,000 to 598.31 per 100,000, a rise of 92.80%, with an average annual increase of 2.55%. The number of prevalent cases increased from 41.04 million to 110 million, a rise of 166.97%, with an average annual increase of 3.61%. The prevalence rate increased from 3488.78 per 100,000 to 7701.69 per 100,000, a rise of 120.76%, with an average annual increase of 3.00%. The number of YLDs increased from 1.34 million to 3.55 million, a rise of 165.32%, with an average annual increase of 3.59%. The YLD rate increased from 113.86 per 100,000 to 249.81 per 100,000, a rise of 119.39%, with an average annual increase of 2.99%. High BMI was the only significant attributable risk factor, with the proportion of YLDs it caused continuing to rise. Predictions for 2035: The number of incident cases is expected to decline slightly from 5.89 million in 2022 to 5.72 million in 2035. The number of prevalent cases is expected to peak at 72.42 million in 2029 and be around 72.69 million in 2035. The number of YLDs is expected to increase year by year, from 2.35 million in 2022 to 2.35 million in 2035. <em>Conclusion</em>: The study reveals the increasing prevalence and disease burden of knee osteoarthritis among Chinese residents, emphasizing the importance of interventions targeting controllable risk factors. The diagnostic efficacy of high-frequency ultrasound was evaluated by comparing it with the results of surgical pathology. <em>Results</em>: High-frequency ultrasound detected 50 benign nodules, primarily breast fibroadenomas, and 35 malignant nodules, mainly breast ductal carcinoma in situ. Based on surgical pathology results, the diagnostic accuracy of high-frequency ultrasound was 96.47%, specificity was 97.96%, and sensitivity was 94.44%. In high-frequency ultrasound diagnosis, the proportion of grade III and IV blood flow in malignant nodules was higher than that in benign nodules, while the proportion of regular shape and clear margins was lower. The proportion of microcalcifications and posterior echo attenuation was higher in malignant nodules, and the resistance index (RI) and peak blood flow velocity were lower than those in benign nodules (P &lt; 0.05). <em>Conclusion</em>: High-frequency ultrasound can effectively differentiate benign and malignant breast micronodules, determine specific nodule types, and exhibits high diagnostic accuracy and sensitivity. Descriptive statistics and visualization tools were used to map the developmental trajectory and research hotspots of this field. <em>Results: </em>The analysis included a sharp rise in publications over the past two decades, with especially rapid growth after 2015. Collaborative networks highlighted a few key countries and research centers driving the field. A comprehensive evaluation of exceedance factors revealed that high-risk factors included silica dust and benzene, classified as Level 4, while medium-risk factors encompassed manganese and xylene, including their compounds, classified as Level 3. <em>Conclusion</em>: After long-term monitoring of multiple workplaces in the city, it was found that there were certain occupational hazard factors, specifically involving dust or noise, as well as related factors of chemical toxins. Among these, silica dust and benzene posed the highest risks, while manganese and xylene also carried relatively high risks. Caregivers were surveyed using a questionnaire regarding family caregiving practices for infants and toddlers. The Kruskal-Wallis’s test, Mann-Whitney test, and multiple linear regression analysis were employed to examine the influencing factors of family caregiving for infants and toddlers. <em>Results</em>: The findings indicate that the quality of family caregiving is significantly influenced by the educational level of the parents (<em>p</em> &lt; 0.001) and the annual household income (<em>p</em> &lt; 0.001). Work-family conflict showed a significant negative correlation with the quality of family caregiving (β = -0.239, <em>p</em> &lt; 0.001), while family atmosphere demonstrated a significant positive correlation with the quality of family caregiving (β = 0.225, <em>p</em> &lt; 0.001).<em> Conclusion</em>: In Tianhe District, Guangzhou City, the quality of infant and toddler caregiving is determined by a combination of key factors, including annual household income, parental educational background, work-family conflict, and family atmosphere. Design: A pre- and post-training evaluation study. <em>Methods</em>: A total of 31 pediatric outpatient nurses participated in the study, completing a self-designed 15-item questionnaire focused on clown therapy. The questionnaires were administered both before and after a structured training program. The assessment aimed to measure changes in nurses’ knowledge and attitudes toward clown therapy. <em>Results</em>: The results indicated a statistically significant improvement in the nurses’ cognition following the training (mean score post-training: 8.55 vs pre-training: 7.71, <em>p</em> = 0.003). <em>Conclusion</em>: Notably, the training also eliminated the age-related differences in cognition observed prior to the intervention.
